A Waist Circumference and Body Fat Percentage Dual-Dimension Nine-Square Grid Model for Precision Obesity Management: A Randomized Controlled Trial in Overweight/Obese University Students
Sponsor: Hainan Normal University
Summary
This single-center, two-arm, parallel exploratory randomized controlled trial compared adaptive multicomponent weight management guided by a 3×3 waist circumference-body fat percentage (WC-BFP) phenotype classification with structured aerobic exercise over 8 weeks. Forty-eight university students with overweight, central obesity, or elevated BFP were randomized 1:1. The adaptive intervention combined aerobic, resistance, and flexibility exercise with phenotype-matched nutritional and behavioral support; WC and BFP were reassessed every 2 weeks to guide prescription adjustments. The comparator involved 3-5 aerobic exercise sessions per week, totaling more than 150 minutes per week, with progressive intensity from 40%-50% to 60%-80% of heart rate reserve. Both groups received individualized minimum daily energy-intake targets, wearable monitoring, online exercise and dietary logging, and remote supervision. Anthropometric outcomes and exercise self-efficacy were assessed at baseline and weeks 4 and 8; exercise adherence was assessed at weeks 4 and 8. The manuscript identifies week-8 body weight as the primary outcome, five additional anthropometric measures as secondary outcomes, and behavioral measures and phenotype transitions as supportive or exploratory outcomes.

Dynamic Matrix Management for Weight Control (DMM-WC)
Precision weight management using a two-dimensional grid model. Participants undergo bi-weekly positioning on a waist risk-body fat matrix (9 grids), receiving tiered interventions (e.g., high-intensity exercise + dietary control for high-risk grids; maintenance plans for low-risk grids). Real-time progress feedback via visual spatial migration maps promotes adherence. Comparator group receives standardized aerobic prescription without dynamic adjustments.
Standard Aerobic Prescription
Personalized progressive aerobic exercise program. Participants receive baseline-adjusted intensity prescriptions (40-90% heart rate reserve), with adherence monitored via wearable devices. NO dynamic adjustments, tiered strategies, or visual feedback are provided throughout the 8-week intervention.
