Therapeutic Exercise and Education for Low Back Pain

Summary
This study evaluates a new treatment program for patients with ongoing or long-term low back pain who receive care at Primary Care Physiotherapy Units. Participants with subacute or chronic non-specific low back pain will be randomly divided into three different groups to compare their progress. The first group will receive an 8-week program of therapeutic exercise and health education delivered entirely online. The second group will receive the exact same exercise and education program, but in face-to-face sessions. The third group will receive standard conventional physiotherapy care. The entire study lasts for eight weeks, and the researchers will follow up with the participants three months later to see how they are doing. The main goal of the project is to measure changes in the participants' daily disability levels. The researchers will also look at other important factors such as pain intensity, fear of movement, anxiety, depression, quality of life, and overall satisfaction with the care received. Measurements will be taken before the study starts, right after the 8-week program finishes, and at the 3-month follow-up. The ultimate purpose of this trial is to find more accessible, evidence-based ways to treat back pain, reduce hospital waiting lists, and improve patient well-being using both digital and in-person care.

Interventions
Therapeutic exercise
This intervention consists in the application of a therapeutic exercise protocol for lumbopelvic and abdominal musculature. The exercises included are summarized as follows: * Core: isometric dead bug, dead bug leg lowering, contralateral dead bug, dead bug with leg abduction and straight arm drop, band-resisted dead bug * Core + abds: fire hydrant, band-resisted fire hydrant and band-resisted fire hydrant on toes * Quadriceps: chair squat, air squat, asymmetric chair squat, lunge and chair squat with heel drive * Gluteus: sun salutation, single-leg deadfoot drag deadlift, skater (extension + external rotation + abduction), standing donkey kick and single-leg dead lift. Exercise will be performed at an effort level of 6-7/10 (rate of perceived exertion), or leaving 3 repetitions in reserve per exercise. Participants will perform 4 to 6 sets of 15 repetitions for each exercise, with 2 sessions conducted per week. Rest periods between sets and exercises will be 1.5 minutes.
Education on self-management of low back pain
Participants will receive 4 weekly low back pain self-management education sessions during the first 4 weeks of the intervention. The contents of the education session will be as follows: * Multifactorial origin of the condition and natural course of the disease * Consequences of rest and benefits of movement * Ergonomic adaptations applicable to daily, work, and sports activities * Interpretation of imaging tests in relation to patient functionality
Usual Care
Participants will receive a total of 16 sessions (usual care), 2 per week. The interventions included will be as follows: * Microwave therapy: 15 minutes, 100 W, continuous * TENS: 80 Hz, 150 microseconds, 20 minutes * Unsupervised exercise program: a general program including mobility, strength, and stretching exercises
