Summary
This prospective, single-center, single-arm interventional study evaluates the safety and preliminary efficacy of a thiotepa-based conditioning regimen in patients aged 50 years or older with severe or very severe aplastic anemia undergoing haploidentical hematopoietic stem cell transplantation.

Interventions
Thiotepa
Thiotepa 10 mg/kg is administered on Day -6.
Busulfan (BU)
3.2 mg/kg/day intravenously on Days -8 to -7.
Fludarabine
Total dose 150 mg/m² intravenously on Days -6 to -2.
Anti-thymocyte globulin (ATG)
Total dose 7.5 mg/kg intravenously on Days -5 to -2.
Basiliximab (Simulect)
Basiliximab 20 mg is administered on Day -1 and Day +4.
