Sponsor: The Hong Kong Polytechnic University

View on ClinicalTrials.gov

Summary

The ENRICH Study is a single-arm, single-site, mixed-methods feasibility and pilot implementation study examining a culturally adapted, nurse-led model of care for social frailty among community-dwelling older adults in one primary health center in Manila City, Philippines. The study comprises four sequential phases: (1) Filipino translation and cultural adaptation of the Montayre Social Frailty-10 (MSF-10); (2) participatory co-design and refinement of the Social Frailty Care Bundle (SFCB); (3) nurse training and competency assessment; and (4) a three-month pilot implementation. It will assess feasibility, acceptability, fidelity, workflow fit, recruitment, retention, follow-up completion, and implementation readiness to inform a future definitive trial.

Social Frailty Care Bundle (SFCB)

A structured nurse-led pathway delivered by trained registered nurses at the health center, in addition to usual care, over 3 months: (1) screening with the Filipino Montayre Social Frailty 10 (MSF-10); (2) domain-based assessment across four social frailty domains (general resources, social resources, social behavior and activity, and self-management); (3) individualized care planning; (4) brief education or coaching using approved materials; (5) referral or linkage, when indicated, to existing health, social welfare, community, faith-based, senior citizens' organization, non-governmental, or local government services (for example, group physical activity or peer support); (6) follow-up contacts, approximately every 4 weeks for lower-intensity needs, with escalation to nurse-led case management for complex needs; and (7) documentation and fidelity monitoring. Bundle content is finalized through participatory co-design before implementation.
