Trochanteric Bursa Repair and Lateral Hip Pain After Hemiarthroplasty for Femoral Neck Fracture
Sponsor: Osman Sarıyar
Summary
This study aims to determine whether repairing the trochanteric bursa during hip hemiarthroplasty reduces lateral hip pain after surgery in patients aged 65 years and older with femoral neck fractures. The trochanteric bursa is a soft tissue structure located over the greater trochanter of the femur. During surgery, this tissue may be opened to access the hip. At the end of the procedure, the bursa can either be repaired or left unrepaired. A total of 102 patients will be randomly assigned to two groups. In one group, the bursa will be repaired using sutures. In the other group, it will be left unrepaired. All other aspects of surgical treatment and postoperative care will follow the same routine clinical approach. The main outcome is lateral hip pain when lying on the operated side at six weeks after surgery, measured using a numerical pain scale from 0 (no pain) to 10 (worst imaginable pain). Additional assessments will examine pain during daily activities, mobility, independence in daily living, and complications during the first three months after surgery.

Interventions
Trochanteric Bursa Repair
Patients will undergo primary hip hemiarthroplasty through a posterior approach. After longitudinal incision of the trochanteric bursa, the bursa will be anatomically repaired using three to four interrupted No. 1 Vicryl sutures. The bursa will not be excised. Capsular and short external rotator repairs will be performed according to the same standard surgical protocol in both groups.
Trochanteric Bursa Left Unrepaired
Patients will undergo primary hip hemiarthroplasty through a posterior approach. After longitudinal incision, the trochanteric bursa will be left unrepaired without excision. All other surgical procedures, postoperative analgesia, mobilization, and rehabilitation will follow the same protocol as the bursa repair group.
