Effect and Cost Effectiveness of a Dyadic Empowerment-based Heart Failure Management Program for Self-care

Global population aging has drastically increased healthcare spending worldwide, with the greatest portion going to hospital and community health services. Heart failure (HF), as the final form of many cardiovascular diseases resulting from insufficient myocardial pumping. Ineffective self-care is consistently identified as the major modifiable risk factor for HF decompensation requiring hospitalization. It refers to an active cognitive process that influence patients' engagement in self-care maintenance, symptom perception and self-care management. However, current studies pay much focus on interventions such as motivational interviewing and behavioural activation to enhance the HF-related self-care and health outcomes which only produces short-term benefits. In fact, the lack of a sustainable effect from the self-care supportive interventions might be related the use of patient-centric design in these studies, which totally ignores the fact that HF management takes place in a dyadic context. To advance, active strategies were adopted to mobilize collaborative effort of the dyad in actual disease management. This study aims to evaluate the effects and cost-effectiveness of a Dyadic empowerment-based Heart Failure Management Program (De-HF) for self-care, health outcomes, and health service utilization among HF patients who require family support after hospital discharge. The De-HF program is based on the Theory of Dyadic Illness Management to enhance the congruence in illness perception and active dyadic collaboration in managing HF via both face-to-face and online platforms.

Delivering Food Resources & Kitchen Skills (FoRKS) to Adults With Food Insecurity and Hypertension

The goal of this clinical trial is to determine the impact of a home-delivered foods and kitchen skills program on health and nutrition in adults with high blood pressure and food insecurity. Researchers will compare Food Resources \& Kitchen Skills (FoRKS) and Enhanced Usual Care (EUC) to evaluate the effects on mean systolic blood pressure (SPB), HbA1c, food security and nutrition. Participants will complete 24-hr blood pressure monitoring, standard blood pressure measurements, weight, finger stick for A1c point-of-care testing, and questionnaires.

Investigation of the Relationship Between Strengthening and Ikigai Levels of Individuals With Coronary Artery Disease

cardiac rehabilitation includes topics such as smoking cessation recommendations, weight management, nutritional counseling, blood pressure management, physical activity counseling, diabetes management, psychosocial management in order to reduce CV risk factors, reduce morbidity and mortality rates, and improve the individual's quality of life . In CV diseases, it has been shown that education and motivation provided under the leadership of nurses are effective in preventing the disease. This success stems from the nurse's holistic approach to the individual and determination of individual-specific needs. It is important for nurses, who are seen as a source of support by the society, to identify empowerment in CV patients and to support patients for independent living. kigai is a well-known term in Japan. This term is sometimes translated as 'purpose in life' and sometimes it can be defined as 'joy of life'. When Ikigai is mentioned, both the source of Ikigai within the individual or the object of Ikigai, and the psychological/emotional state (Ikigai-kan) in which Ikigai is felt come to mind. Accordingly, 'Ikigai' can be defined as the source or object of the purpose/joy of life, and 'Ikigai-kan' as the psychological/emotional state in which these are felt. In addition to the ways in which individuals with cardiovascular disease manage their disease, take precautions against risk factors, and perform strengthening exercises, determining the patients' current state of mind is also an important element. In this sense, physicians and nurses in the clinic take these elements into consideration when planning the care to be provided to patients. No study has been found in the literature to date that evaluates the relationship between CAH strengthening and Ikigia. Based on this information, this study aims to examine the relationship between CAH strengthening and Ikigai. It is expected that the research results will guide physicians and nurses who care for individuals with CAH in planning the care to be provided to patients and contribute to the literature.

Feasibility Study of a Relapse Investigation Questionnaire Administered by an MS Care Coordinator Nurse in Patients With Multiple Sclerosis

This study aims to evaluate a questionnaire used by a specialized nurse to help manage relapses in people with multiple sclerosis (MS). The questionnaire is completed during a phone call when the patient reports new or worsening symptoms. It helps the nurse decide whether further tests or treatments are needed, based on a protocol developed with a neurologist. The goal is to improve how quickly and effectively patients receive care during a relapse. The study will include 10 adult patients with active forms of MS over a two-month period.

The Effect of the Training Programme Based on the Health Promotion Model Given to Parents

This randomised controlled study evaluates the effect of a training programme based on the Health Promotion Model developed for parents of children diagnosed with epilepsy on parents' level of knowledge about epilepsy, parents' general self-efficacy level, parents' health promoting and protective behaviours and the number of hospital admissions of their children. The hypothesis of this study is that education has an effect on these.

Evaluation of the Chronic Disease Co-Care Pilot Scheme

The Chronic Disease Co-Care (CDCC) Pilot Scheme is initiated for early screening and management of people with hypertension (HT), pre-diabetes mellitus (pre-DM) and diabetes mellitus (DM) in Hong Kong. This study will evaluate the quality of care, feasibility, acceptability, effectiveness and cost-effectiveness of the Scheme. This is a 12-month cohort study among the District Health Centre (DHC) or DHC Express, healthcare providers, CDCC participants and a comparison group of 1,886 non-participants. All the person-in charge, 2 family doctors and 1 of each allied health provider from each DHC/DHC Express will be administered with the questionnaires on quality of care and costing. All CDCC participants will be included in subject characteristics, among which a convenience sample of 548 will complete a telephone survey on experience, enablement and satisfaction of the CDCC Pilot Scheme. The health outcomes of 1,886 CDCC participants and 1,886 non-participants will be compared for evaluation of effectiveness and cost-effectiveness. Participant characteristics, enablement, compliance to the standards of care, and costing of CDCC Pilot Scheme will be summarized using descriptive statistics. Differences in the proportion of patients meeting treatment targeted for HT, pre-DM and DM after 12 months will be compared by chi-squared test and logistic regressions. The incremental cost-effectiveness ratio will be evaluated by comparing with the World Health Organization (WHO) threshold. This study will inform future healthcare planning and policy for manpower and resource allocation.
