NURsing-led FRAILty Prevention and Care

The goal of this quasi-experimental study is to learn if a nurse-led care program can help prevent or manage frailty in older adults living in the community. The study focuses on people aged 65 years and older who have at least one chronic condition, show early signs of frailty, and have difficulty with self-care. The main questions it aims to answer are: * Does the nurse-led NUR-FRAIL program improve participants' ability to care for their chronic conditions after six months? * Does the program improve quality of life and reduce health-related difficulties? * Does the program lower emergency department visits and hospital stays over one year? Researchers will compare participants who receive the NUR-FRAIL program with participants who receive usual care to see if the program leads to better self-care, better quality of life, and less use of hospital services. Participants will: * Meet with a Family and Community Nurse for health and frailty assessments at the start of the study, after three months, and after six months * Complete short questionnaires about self-care, daily functioning, and quality of life * Receive either usual care or a three-month nurse-led program that includes personalized education, goal setting, lifestyle advice, and support for managing chronic conditions * Allow researchers to collect information about emergency visits and hospital stays for up to twelve months Some participants in the nurse-led group will also be invited to take part in an interview to share their experience with the program.

Turkish Adaptation of a Sleep Screening Tool for Pediatric Complex Chronic Conditions

This study aims to adapt the Sleep Screening Scale for Children and Adolescents with Complex Chronic Conditions (SCAC) into Turkish and test its reliability and validity for this population. Children and adolescents with complex chronic conditions (CCC) often have ongoing and multiple health problems. These children often face a high risk of sleep problems, but there are no screening tools in Turkish designed specifically for them. The main goal is to confirm that the Turkish version of the SCAC is accurate and reliable. The study will also look at how common different types of sleep problems are in children with CCC, and how these problems are related to factors such as diagnosis, age, sex, and other medical conditions. Another goal is to compare children's sleep at home with their sleep during a stay in the pediatric intensive care unit (PICU). The study will also examine how environmental factors (such as light and noise) and medical factors (such as pain, medications, and devices) affect sleep in the hospital. After discharge, sleep recovery will be followed for up to 3 months using sleep diaries and actigraphy (a wearable device that measures movement during sleep).

Evaluating Covered California's Grocery Support Program

This is a pragmatic randomized controlled trial (RCT) of Covered California's Grocery Support Program. This pragmatic RCT will test the efficacy of providing a monthly food card benefit for adults with food insecurity, a chronic condition, and incomes below 250% of the federal poverty level (FPL) compared with a group who receives a lump sum payment.
